The Housing Navigator reports to TAY (Transitional Age Youth) CES (Coordinated Entry System) Regional Coordinator or their designee and will assist with day to day operations. Peer Navigator will arrange, coordinate, monitor, mentor, and assure all delivered services by the CES team meet the needs of TAY RRH/TVFS. To be successful in the position one must be motivated with a sense of teamwork and possess good communication and people skills.
DUTIES & RESPONSIBILITIES
Assist Program Coordinator with screening daily referrals of TAY RRH/TVFS and CES housing programs.
Designates incoming cases for intake to case management team and ensure case management collaboration to ensure stabilization and more permanent housing placement .
Conducts comprehensive file reviews and assessment of data collected to assure functional, environmental, psychosocial, financial, employment, housing, educational, and health information is appropriate to develop a case plan.
Develops support systems with case managers to meet youth household needs by identifying and coordinating a variety of available services necessary to maintain independent living, and, when possible, self-sufficiency and family stabilization.
Assures all necessary documentation is collected by case managers for RRH/TVFS services verification purposes.
Provides support to Case Managers/CES, as well as ongoing counseling and advocacy to all veteran households.
Makes referrals to available TAY/Young Adults community providers for therapeutic, social, educational, and medical resources y building community partnerships.
Conducts crisis intervention as necessary.
Collaborates with Case Managers to assure all available services are provided to each RRH/TVFS household on a regular basis, determining quality and effectiveness of services provided.
Refer TAY/Young Adults households to Case Manager/CES for intakes and ensures all supporting documents are verified/files.
Review cases with CES Coordinator for RRH/TVFS households requiring emergency assistance.
Assure all RRH/TVFS assets and file documents are maintained and secure for all youth households.
Monitors 90 day recertification documents progress of RRH/TVFS households.
Administers direct assistance of funds by way of third party check payments.
Provides information and referrals for callers and walk-in RRH/TVFS households.
Develops, performs, and assists with workshops pertaining to RRH/TVFS households needs.
Must be able to deliver the Next Step Tool (NST) to all youth who are in need of housing services.
Assist and support CES spa wide
Co-lead Youth Community Advisory Board (YCAB)
Participates and assists with outreach events.
Attend and when needed conduct weekly staff meetings.
Attend regularly scheduled agency wide meetings, community meetings and other meetings as assigned.
Conduct all functions in accordance with Agency guidelines, policies and procedures.
Comply with applicable certification requirements and ethics.
Arrange work schedule or required hours to the benefit of the Department/Program with the approval of immediate supervisor.
REQUIREMENTS & QUALIFICATIONS
- Bachelor’s degree from an accredited college or university preferred.
- Three (3) plus years’ experience working as a Case Manager or Peer Navigator with the TAY/Young Adult homeless population.
- Ability and desire to work with people from various backgrounds and/or limited English capabilities.
- Demonstrated proficiency in MS Word, Excel, and internet usage.
- Database skills (e.g., HMIS) strongly preferred.
- Able to lift up to 25 pounds.
- Valid California driver’s license and insured automobile.
- Bi-lingual English/Spanish preferred
- CES/ LAHSA experienced preferred
